CORE REQUIREMENTS: New or First Application for an ePassport

3.Proof of Philippine citizenship plus one photocopy of the document/s

Born in the Philippines: Philippine Statistics Authority (PSA) birth certificate
Born overseas: PSA report of birth

Note:
i. Those who filed their report of birth with the Philippine Consulate in Los Angeles have one year from the date of registration to apply for a passport without presenting a PSA-copy of the report of birth.
ii. The local civil registrar copy of the birth certificate or report of birth is required when the details on the PSA copy are difficult to read.

For those who elected PH citizenship:
PSA birth certificate plus Election documents and Identification Certificate issued by the Bureau of Immigration

For naturalized Filipino citizens, please call 213-637 3018 or email phppt.lapcg@gmail.com for the documentary requirements applicable to you.

4. US permanent resident card
Note: For dual citizens, please refer to Dual Citizens under RA 9225

If you do not have a permanent resident card yet, present valid US visa and one  valid government-issued ID

5. For new ePassport applicants whose last passport was a non-electronic passport, present the said passport and one photocopy
6. Proof of payment of passport fee (please refer to EPASSPORT FEES AND OTHER INFORMATION)
7. Self-addressed USPS Priority Mail or Priority Express mailing envelope with prepaid postage
