Additional Requirements

Follow the core requirements for ePassport renewal or new/first ePassport application plus the following:

Dual Citizens under RA 9225

1. Dual citizenship documents plus one photocopy per document:
a. Order of Approval;
b. Oath of Allegiance; OR
c. Identification Certificate or Certificate of Retention/Reacquisition of Philippine Citizenship

2. Valid government-issued identification card

Married Women

Follow the core requirements for ePassport renewal or new/first ePassport application and present the original and one photocopy of the following documents, as appropriate:
1.  If adopting the husband’s surname
PSA marriage certificate OR PSA report of marriage

Note:
i. Those who filed their report of marriage with the Philippine Consulate in Los Angeles have one year from the date of registration to apply for a passport using their husband’s surname without presenting a PSA-copy of the report of marriage.
ii. The local civil registrar copy of the marriage certificate or report of marriage is required when the details on the PSA copy are difficult to read.

2. If reverting to their maiden name
PSA marriage certificate with annotation on the annulment of the marriage plus the certificate of finality issued by the concerned court, OR PSA report of death of spouse

Note: The local civil registrar copy of the marriage or death certificate is required when the details on the PSA copy are difficult to read.

Minors

1. Refer to the core requirements for either ePassport renewal or new/first ePassport application.
2. If the minor is a dual citizen at birth (both Filipino and US citizen) born in the Philippines, present Consular Report of Birth, if applicable, and US passport plus a copy of each document.
3. If the minor is a dual citizen at birth (born to Filipino citizen parents in the US), a report of birth is required. Present the PSA copy of report of birth, or the local copy of the report of birth if registered less than a year from application of a new ePassport, and US passport plus a copy of each document.
4. Minor must be accompanied by either parent (if the child’s parents are married), by the mother (if the parents are not married), legal guardian or authorized adult companion.
5. The child’s PSA birth certificate or PSA report of birth is always required to show proof of filiation.
6. If the accompanying parent is not a Filipino citizen, the latter must present his/her foreign passport plus a copy of the data page and a copy of the Filipino spouse’s Philippine ePassport or other valid competent proof of identity.
7. If the minor is accompanied by an authorized adult, a Special Power of Attorney (SPA) executed by either of the minor’s parents (if married), by the mother (if the parents are not married), or the child’s legal guardian, as the case may be, to be presented with the adult companion’s passport or other competent proof of identity, and a copy of the passport or other competent proof of identity of the minor’s parents.

Lost or Damaged ePassport

Follow the core requirements for ePassport renewal and present the original and one photocopy of the following documents, as appropriate:
1. Lost Passport
Notarized affidavit of Lost Passport explaining how the ePassport was lost.
Note: The applicant may present an affidavit notarized by a licensed notary public or they may lodge their affidavit with the Consulate as part of the reporting and replacement process.

If the loss is due to fire or theft and the incident was reported, present the police report on the incident

If the loss is due to mail handling and the applicant did not receive his/her ePassport, the applicant must first liaise with USPS to confirm delivery or non-delivery of the package containing the ePassport and present the USPS certification in support of the reprinting of the replacement ePassport.

If more than six months has passed before the loss of the ePassport package was reported to the Consulate, the replacement will be treated as a regular lost passport.

2. Damaged or mutilated ePassport
Notarized affidavit explaining how the ePassport was damaged/mutilated.
Note: The applicant may present an affidavit notarized by a licensed notary public or they may lodge their affidavit with the Consulate as part of the reporting and replacement process.

Renewing en ePassport with Incorrect Biographical Data

Follow the core requirements for new/first issuance of an ePassport plus the following:

Notarized affidavit detailing the circumstances resulting to the incorrect biographical data in the ePassport

Note: The applicant may present an affidavit notarized by a licensed notary public or they may lodge their affidavit with the Consulate as part of the reporting and replacement process.
